Charles City, IA – Medicaid fraud has led to a Charles City man being arrested.
56-year-old Michael Koster was taken into custody on Monday around 5 p.m. He allegedly forged signatures on Medicaid forms. He did this five times that resulted in him getting paid $748.24. These forgeries happened between Dec. 18, 2016 and Jan. 1, 2017.
Koster was arrested after a traffic stop on North Grand Ave. and Clark St. There had been a warrant issued for his arrest. He has been charged with forgery, third-degree fraudulent practice and tampering with records.
